This service is short term care at home to aid recovery after discharge from hospital. For example, if someone has had a fall that led to hospital admission, they may not be physically capable of coping with daily domestic tasks. Short term care at home may also involve physiotherapy, occupational therapy, and nursing care. Relying on nurses in hospital can affect an individual’s confidence so reablement is a better alternative, as the person can be encouraged to be more independent in a familiar environment.
